Special dietary requirements (school dinners or packed lunches)
If your child has a special dietary requirement, you must complete the below form at the start of each academic year. You need to do this regardless of whether your child has a school or packed lunch.
The following points are very important:
- Olive Dining require an allergy form to be completed (please see below.)
- Olive Dining require medical evidence to be provided for all children with allergies and intolerances in order for them to provide your child a meal.
- If you do not provide medical evidence supporting your child's allergy Olive dining will only offer your child a jacket potato and suitable topping until this paperwork is completed.
